The concept works but it need a bit more of polish here and there. (I undestand that it's a 24H game and that it's a limitation itself) In my first playthough I didn't get it, I did in the second, after reading the instruccions and context, so:
A short text with the controls at the beggining would ne nice (At my first, I though I had to make many short clicks instead of holding and relasing for example, so my heart was moving weirdly)
I bumped in the black hearts really quickly maybe that was one me for, moving the heart weirdly, but I'd suggest to decrease the number of them at the start and add more and more as time pass on.
I would get rid of the screen reload when you get hit, it make it seems like a restart and I thing it's not the narrative of the game to get hit an start again but, to get hit and continue going on in the same hostil space till you break up and grow up the courage to override the spikes for yourself.
Grafics (but the epilepsy ending) and sounds are fitting the theme, so thumbs up.

Sure.
When you get hit, the screen turns red and you get relocated at the center of the stage. That makes the impression that you have lost the level and have to start over (when actually it the thing that was supossed to happen) so, if there was just the red thing but not being relocated, it will be make a sense of continuity rather than a retry.
